Your contact information (previously on the first page of the questionnaire) will now be completed and/or confirmed online when you submit your documents. Before submitting, please select “Questionnaire” from the drop-down menu and complete the requested information (or simply confirm it).

After you log in to retrieve your forms, use the buttons at the top—“Download Census” and “Download Questionnaire”—to download the documents to your computer for completion. Once complete, log in again to upload your documents; directions for uploading are provided on that page.
